Adding Custom Fields to a Calendar Appointment

I have a manager asking for us to created a shared or public calendar for our IT department to use to schedule travel time. The manager would like for the ability to select from predefined fields in a drop down list when creating the appointment that are standardized to everyone who has access to the calendar.

A sharepoint site is out of the question; the manager wants to be able to add the Calendar to Outlook. I have researched this issue and have not found a definitive solution. Is there a third party application that would allow me to do this?

Is there a third party application that makes it simple do this? I'm not intricately familiar with Outlook.
I am not aware of any third-party applications that simplify creating a custom Outlook form.

After a custom form is created, would it be possible to make it to where whenever a new appointment is added to the calendar, the new appointment defaulted to the custom form for everyone who had access to that calendar?
Yes, that's possible.  I believe there are instructions for doing that on the page I linked to in my initial response.
